Kindergarten Immunizations - Is your child prepared?

Immunization requirements are specific in the state of Minnesota for elementary, middle, and high school students.  Parents must show proof of required shots for a child to attend school:  No Shots, No School.  Parents may file a medical exemption signed by a health care provider or a conscientious objection signed by parent/guardian and notarized.  For more information on specific requirements, click on the following for ISD 192 Immunization Requirements and Dakota County Family Health Services available to families living in Dakota County.

Before starting kindergarten, all students must have the following immunizations:

  • DTP (diphtheria, tetanus, pertussis):  5 doses
  • Polio: 4 doses
  • MMR (measles, mumps, rubella): 2 doses
  • Hepatitis B:  3 doses
  • Varicella (Chicken Pox):  2 doses
